Living Tribute Name Changes

Please read before completing the following form:
If you have used Hamilton's Funeral Home to provide services for your loved one, you should have received a letter explaining our Living Tribute program, offered in honor and remembrance of those who we've served in partnership with the Des Moines Botanical and Environmental Center. This letter is sent within 8 weeks after the time of death and indicates that way in which your loved one's name will appear on the Botanical Center display as well as in our Living Tribute books at our Hamilton's Funeral Home locations. If you would like your loved one's name to appear differently than how it is listed in your letter, please fill out the form below.
